As part of your reopening plans, you can pre-screen students and determine whether or not they can come to campus.
Based on a student’s answers to the pre-screener form, you can update a student’s Health Pass in the campus app. You can give a green Health Pass, meaning cleared to come to campus. Or you can give a red Health Pass with instructions on what to do next.
Here are the steps you will take to set up daily pre-screening:
- Make a daily pre-screening form using a template
- Put the pre-screening form into the campus guide

- Preview the experience / Inform students
Make a Pre-Screener Form Using a Template
Students will answer questions that are grouped together into a form. By answering questions and submitting the form, students give you the data you need to decide whether they should be admitted to campus, or told to stay home.
First, create a form with questions students will answer. Contact Trace Plus includes three forms you can use as a template, including “Daily Pre-Screening.” Later, you can set a daily reminder that will appear in each student’s campus app, each morning Monday through Friday.
Follow the steps in this article and select the “Daily Pre-Screening” template:
You can use the template with no modifications. The pre-screening template is based on CDC guidelines and we recommend using it without modifications for now. If you do wish to change the form, please see:

Put the pre-screening form into the Campus Guide
When you are done making any changes to the pre-screening form, click “Publish.” You are now ready to share the form.
You will want to share the pre-screening form using the campus guide, where it is easily accessed in the mobile app by students and visitors to campus. You will use Campus Cloud Studio to create a tile in the campus guide, linked to the form. You will access the form by tapping the tile.
The campus app uses “Experiences” -- for example, students, alumni, and visitors -- to show different information to different people. If you want a form to be available to those different groups, be sure to add your form to each experience where you want to share.

You can place the daily pre-screener at the top of the Campus Guide by making it a
featured tile. As a featured tile, the daily pre-screener will appear at the top of the campus guide. Here is how you can set it up:
Set up a daily reminder
You can now set up notifications or reminders for students to fill the form each day before coming to campus. The reminder for a form is sent at 7:00 AM in your school’s time zone, Monday to Friday.
Students receive the notification in the Notifications Center of the campus app, where they can respond and answer your form.

If a student uses the campus app to scan the QR code and fill out the form, two additional things happen:
- Their Health Pass is updated based on the answers to the questionnaire;
- An entry is created in the student’s Health History, recording the student’s answers to the questions
