As part of your reopening plans, you can create a symptom tracker to log the daily symptoms of students. You can track the symptoms of students coming to the campus by creating a Symptom Tracker form and asking them to fill it. This will give you and the students a good idea of the medical status of the student and help you decide if they should be allowed to attend campus or not.
Here are the steps you will take to set up a symptom tracker.

- Share that form
- Put that form into the campus guide

Make a symptom tracker form using a template
Students will answer questions that are grouped together into a form. By answering questions and submitting the form, students give you the data you need to decide whether they should be admitted to campus, or told to stay home.
First, create a form with questions students will answer. Contact Trace Plus includes three forms you can use as a template, including “Symptom Tracker.” You can use this form.
Follow the steps in this article and select the “Symptom Tracker” template:
You can use the template with no modifications. The symptom tracker template is based on Centers for Disease Control (CDC) guidelines and we recommend using it without modifications for now. If you do wish to change the form, please see:

Share the symptom tracker form
When you are done making any changes to the symptom tracker form, click “Publish.” You are now ready to share the form.
After publishing, your form is available on the internet. You will use a computer with an internet connection to display the published form at a help desk to manually check students in. Click the “Share” button to copy the address of the published form, and send it (by email, USB key or by other means) to the help desk staff.

Put the symptom tracker form into the Campus Guide
You will want to share the symptom tracking form using the campus guide, where it is easily accessed in the mobile app. The campus guide can be viewed by students and visitors who can then fill in their current symptoms.
You will use Campus Cloud Studio to create a tile in the campus guide, linked to the form. Students will access the form by tapping the tile.
The campus app uses “Experiences” -- for example, students, alumni, and visitors -- to show different information to different people. If you want a form to be available to those different groups, be sure to add your form to each experience where you want to share.

You can place the Symptom Tracker form at the top of the Campus Guide by making it the
featured tile. When students log in to this section of the app, it will appear at the top of the campus guide. Here is how you can set it up:
Set up a daily reminder
You can now set up notifications or reminders for students to fill the form each day before coming to campus. The reminder for a form is sent at 7:00 AM in your school’s time zone, Monday to Friday.
Students receive the notification in the Notifications Center of the campus app, where they can respond and answer your form.
